• I waive and release EVW Midget Football, Elkhorn Public Schools, Firth Public Schools, Springfield Public
Schools, Plattsmouth Public Schools, Nebraska City Public Schools, Missouri Valley Public Schools, City of
Omaha Parks & Recreation Department, Coaches, Officials, Directors, EVW Board Members, Arlington Public
Schools, Omaha Gross Catholic Schools, Omaha Roncalli Schools, Yutan Public Schools, Gretna Public
Schools, Bennington Public Schools, Blair Public Schools, Omaha Public Schools, Millard Public Schools, Fort
Calhoun Public Schools, St. Patrick’s Church, Auburn Public Schools, Malcolm Public Schools, Valley Public
Schools and the Archdiocese of Omaha of any liabilities, as well as for any and all injuries suffered by my child
during the 2008 season.
• I also agree to return upon request, the uniform and other equipment issued to my child in the same
condition it was given, except for normal use and wear.
• EVW MIDGET FOOTBALL LEAGUE REFUND POLICY – A $20 administration fee will be charged on all
refunds. No refunds will be issued after the first 2 practices in pads have been completed.
• We have read the above information and fully understand and agree to the above terms and conditions.
